Position Title:  Case Manager Assistant     

Department:  Case Management

Position Type: Full-time

Supervisor: Case Manager Assistant Supervisor

Supervises: N/A

Exempt/ Non-Exempt: Non-Exempt

 

Position Overview:

This position involves a variety of duties in support of Case Managers.  This position gathers, documents and manages IP and client information using various computer platforms. This position reports to the Case Manager Assistant Supervisor.

 

Essential Functions and Responsibilities include the following:

  • Provide direct assistance to providers and clients at the direction of and on behalf of the Case Manager by phone or in person.
  • Assist walk-in Case Management clientele .
  • Assist clients to obtain equipment and supplies at the direction of the Case Manager.
  • Conduct home visits at direction of the Case Manager.
  • Complete documentation of Service Episode Records (SER) in CARE.
  • Authorize services using the ProviderOne system.
  • Perform data management, editing and report generation within agency and governmental data systems.
  • Track and distribute documents and forms following program policy.
  • Utilize databases to maintain and track work flow and provide reports to Case Management staff.
  • Maintain Case Management general forms inventory.
  • Other duties as assigned.

 

Physical Demands

  • Ability operating a personal vehicle driving to home visits (within Spokane County) as needed using personal vehicle.  Demands average 150 to 300 miles per month depending on client needs.
  • Skilled in typing, keyboarding includes up to 7.5 hours per day.
  • Exhibits manual dexterity for effective operation of vehicle controls and typing. 
  • Ability to lift and carry objects up to 20 lbs., including laptops and client files, includes abilities to push, pull, stretch, reach, and maneuver in various directions. 
  • Ability to sit, stand and walk, including navigating stairs and uneven terrain, while utilizing bending, stooping, and climbing as needed to access materials.
  • Maintains visual acuity at various distances and operates in diverse environmental conditions, including both indoor and outdoor noisy settings while navigating safely.
  • Speech and listening skills are sufficient to interact with employees and the public.

Skills, Knowledge and Abilities

Knowledge of aging and long-term care services, including the Area Agency on Aging service network.  Basic skills in MS Word, Excel and Outlook. Requires ability to work independently and as part of a team under deadlines, including the ability to understand and follow verbal and written direction, conceptualize tasks, and efficiently complete a variety of work assignments as delegated.  Demonstrated skills, knowledge, and abilities required in standard office procedures Knowledge of state codes and regulations pertaining to Case Management of community based in-home clients.  Proficiency in a second language helpful but not required.

Minimum Qualifications

Case Aides must have one year of in-person human services experience and a high school diploma or its equivalent. Experience may be paid or volunteer.

Applicants meeting the skills, knowledge and abilities previously listed are preferred. Must pass a criminal background check, post offer.  Must have and maintain a valid driver’s license.
